Mapi Research Trust officially and exclusively distributes all electronic versions of the questionnaires developed by Professor E. Juniper and gives permission on behalf of QOL Technologies Ltd (QOL Tech).
A license agreement must be completed and a User fee is required to all users (commercial & academic users).
Step 1 - Firstly, QOL Technologies Ltd. (Mrs Jilly Styles - jill@qoltech.co.uk) has to be contacted in order to pay a standard one-off user fee calculated per language and per package of questionnaires (commonly called the “paper & .pdf versions”). This package includes the most frequently used versions of questionnaires, an implementation document which provides information on administration, analysis and interpretation, and some reprints that describe the development and validation of the questionnaires. Once this fee has been paid it entitles your organisation worldwide (study sponsor) to unlimited usage of the paper and .pdf versions of the questionnaire(s).
This payment is required even if you intend using the questionnaires electronically.
Step 2 - Please fill out our user agreement and return it to us by e-mail. At this stage, though you are not required to sign the form, the information in the agreement is helping us calculate the quote (access fees) and determine the work that needs to be done with Mapi Language Services. We will be pleased to contact them on your behalf to obtain a quotation for the required work in case no electronic version is available in the language you are requesting and needs to be developed.
Step 3 - If you decide to go ahead, you will be requested to sign the full study-specific user agreement and return it completed to Mapi Research Trust (e-mail and hardcopy for our records). The invoice will then be issued.
Step 4 – If/When the version is available and once your payment has been received by Mapi Research Trust, you may use (depending on your request):

Examination copies can only be used for the limited purpose of examining the suitability of the Questionnaire for subsequent research and/or clinical use, and cannot be used in research or in clinical practice or distributed to others.
You are not authorized to modify, retype, translate, copy or otherwise duplicate the Questionnaire except with the further and prior written permission of Mapi Research Trust / the developers / copyright holders / distributors.

In order to meet the Owner’s requirements of ensuring the validity of the Questionnaire, the Owner requires that the following processes and quality controls are performed on any new electronic version of the Questionnaire developed by or for the User before being used by the User. For doing so, guidelines and models developed by Mapi Research Trust and approved by the Owner will be provided by Mapi Research Trust to the User, in the form of an “e-Booklet” and must be followed.
The Questionnaire e-Booklet, developed in collaboration with the Owner, is intended to accompany the User through the development of official, validated, and approved electronic versions of the Questionnaire. It includes instructions and checklists on instrument properties and configuration, content, navigation, workflow diagram, format and provides a model about how the electronic version of the Questionnaire must be displayed (i.e. the “Original electronic Versions” (“OeVs”) of the Questionnaire).
The following describes the development and validation process for new electronic implementation of the COA:
After implementation of the Questionnaire on the device(s) by the User (or the IT Company on behalf of the User) using the e-Booklet instructions and checklist, screen captures will be generated as displayed on the device(s). These will be reviewed by Mapi Research Trust to check that they are consistent with the instructions provided in the Questionnaire e-Booklet. Corrections that may be needed will be reported to the User. In this case, screenshots after correction will be generated for another round of review until all screenshots are approved by Mapi Research Trust.
The review of the screenshots is mandatory for any new implementation of the Questionnaire on any new device(s) and should be performed exclusively by Mapi Research Trust with a specific budget and contract. Certificate of conformity will be produced by Mapi Research Trust.
After implementation of the Questionnaire into the device(s) and the screenshot review is completed, the User will provide Mapi Research Trust with a test device that will be reviewed and tested by Mapi Research Trust and/or Mapi/ICON Language Services to check that it is consistent with the instructions provided in the Questionnaire e-Booklet. Corrections that may be needed will be reported to the User. In this case, another round of review may be needed until device(s) is approved by Mapi Research Trust.
The Expert Review is mandatory for any new device(s) or modalities and should be performed exclusively by Mapi Research Trust and/or Mapi/ICON Language Services with a specific budget and contract. Certificate of conformity will be produced by Mapi Research Trust and/or Mapi/ICON Language Services.
Usability Testing with subjects shall the be conducted by Mapi/ICON Language Services using standard protocol of Usability Testing which includes testing the Questionnaire on the device(s) with 5 to 8 healthy subjects for the PRO questions (and with 5 clinicians for any clinician question) to assess the overall usability of the solution, including clarity, navigation, font size, and device usability.
The Usability Testing is mandatory for any new device(s) or modalities and should be performed exclusively by Mapi/ICON Language Services with a specific budget and contract. Certificate of conformity will be produced by Mapi Research Trust and Mapi/ICON Language Services.
When the e-Booklet instructions are followed and respected, the User (or the IT Company on behalf of the User) are relaxed from performing any quantitative equivalence studies.
In case the User (or the IT Company for the User) wanted to develop an electronic version of the Questionnaire that would not follow the e-Booklet instructions and models, it is required by the Owner that all such original e-versions are subject to quantitative equivalence studies by ICON Patient Centered Outcomes experts (“ICON PCO”) exclusively. The objective of the quantitative equivalence study is to provide evidence that the electronic version is providing valid and equivalent data to the paper version.
The quantitative equivalence study should be performed exclusively by ICON PCO with a specific budget and contract. Should an original e-version not provide valid data following the above mentioned quantitative equivalence study, permission for use may not be granted by Mapi Research Trust and a user agreement cannot be signed.
The existing Questionnaire’s translations developed for paper administration will be provided to the User or the IT Company on behalf of the User. They will be updated by Mapi/ICON Language Services to reflect the changes in wording of instructions of the original eversion if applicable, during the population into the technical files of the device(s) by Mapi/ICON Language Services.
All translations that are implemented from paper format to electronic format are subject to a screenshot review. The objective of this screenshot review is to check the format and consistency with the paper validated translation and with the e-Booklet OeVs. Provided that the original/source electronic version of the Questionnaire is approved in the device, validation of translated versions can be limited to screenshot review.
The screenshot review of the translations is mandatory and should be performed exclusively by Mapi/ICON Language Services with a specific budget and contract. Certificate of conformity will be produced by Mapi/ICON Language Services.
