- Child Edition (CHIP-CE)
- Adolescent Edition (CHIP-AE)

- The CHIP-AE contains 107 items plus 46 additional optional items specific to disease or injuries.
- For the CHIP-CE, there is a Child Report Form (CHIP-CE/CRF) and a Parent Report Form (CHIP-CE/PRF). The CHIP-CE/CRF has 45 health items and 4 demographic items. The CHIP-CE/PRF has the same 45 health items as the CRF and 7 demographic items, whereas the CHIP-CE/PRF-76 has an additional 31 health items (total 76). An additional Disorders module is available for all PRF versions that allows parents to identify the child's acute and chronic conditions (41 items)
